Prior to scrolling down to the form, please see below as your inquiry might be better suited for a different form. Or, your question may already be answered on another page on our website.
- If you have questions about becoming a member of Faithful Paws, please visit this page. If you have additional questions or need assistance, please use the form below.
- If you are a new facility interested in having ongoing or one-time visits (i.e. for a special event) from a Faithful Paws Team, please apply via this page.
- If you are an approved facility and would like to request additional visits (weekly, bi-weekly or once a month), please use the Ongoing Visit Request Form.
- If you are an approved facility and would like to make a change to your regularly scheduled visits, please email us here.
- If you are an approved facility and would like have a Faithful Paws Team come for a one-time visit, please complete our One-Time Visit Request Form. Please note that we need several weeks advance notice to organize our teams. Holiday and end-of-semester visits book up early, so please send in your request as soon as possible.
