Was it easy to understand the requirements for membership and the application process on our website? Please be specific if any areas were unclear.

For the "Invitation to Train" email which you received from the Executive Director after your application was approved, please tell us if (1) the instructions were clear, (2) you knew what was expected, and (3) you understood where the training class was held and where to park. Please be specific if there are were areas that were unclear.

Regarding the communication from our Training Coordinator before class began, (1) did you copy the contact information for the trainer, (2) were the instructions about payment to the trainer clear, and (3) did you require any information that was not provided?

Was the trainer for classes 1, 2, 3 and 5 friendly and helpful and did she address your specific needs? Please provide feedback below:*

Thinking about Class 4, "Visit 101", what particular parts of the class were helpful (multiple communications practices, accessing the Member Handbook, handbook quiz, pet treat lesson, wheelchair lesson, hallway lesson, elevator lesson, stairs lesson, vest sizing, etc)? What other lessons would be helpful to a new member?
