Team Leaders: Please let us know who is on your team(s) by completing the form below. We need to know where and when our members are visiting in case there is ever an issue with a pet or handler that requires our attention.
The form allows you to add information about more than one team/visit. Once you complete the information for one team, you can add information for a 2nd, 3rd, etc. team by choosing YES to the question, “Do you have another visit and team to add?”.
If you visit the same facility on multiple schedules (i.e. on Tuesdays at 2pm and Thursdays at 4pm), and the team is exactly the same for each visit, please list the team only once and indicate the different visit schedules in the “Day and time of visit” field.
If you visit the same facility on multiple schedules and there are different team members for each visit, please list these visits and teams separately by choosing YES to the question, “Do you have another visit and team to add?”
